Elevated Therapy accepts Aetna, CareFirst, and United/Optum.

Out of Network

We've partnered with Mentaya, a service that streamlines getting reimbursed for your therapy sessions through out-of-network benefits.

Mentaya is perfect if you:

  • Have out-of-network benefits

  • Feel overwhelmed by superbills and insurance

  • Simply want to skip the hassle of paperwork!

Here's how it works:

1. Sign up for Mentaya: mentaya.com/c/elevatedtherapy

2. Our practice will enter your sessions into the platform.

3. Mentaya submits the claim and handles any insurance follow-up.

4. You get reimbursed by insurance! Mentaya charges a 5% fee per claim, which includes handling any paperwork required, dealing with denials, and calling insurance companies.

It's risk-free. They guarantee claims are successfully submitted, or a full refund of their fees.

Payment Information

Elevated Therapy accepts debit, credit (including CareCredit), & HSA/FSA cards for payment. Please check with your individual HSA/FSA plan around coverage for services and any potential gaps for eligibility.

Full payment is due at the time of your appointment. For seamless transaction, a credit card must be stored on file.

Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate of the cost of services. This federal law protects you from unexpected medical bills and ensures that if you're uninsured or not using insurance, you’ll receive an estimate in advance for the total expected costs of your treatment.

If you schedule a service at least 2 business days in advance, you’re entitled to receive your Good Faith Estimate  before your session. This estimate outlines the fees for services and helps you plan your care with full financial transparency.

Cancellation Policy

At least 24 hour notice is required to cancel or modify an appointment, otherwise it will result in a $125 fee for 50 minute sessions or 50% of therapy intensive service. This policy applies to:

  • Intake evaluation

  • Late arrivals (being 15 or more minutes late to a scheduled appointment

  • Missed appointments

  • Late modification requests with less than 24 hour notice
